• Home Medical Service – attaches
a doctor to closely assess, manage and monitor patient’s health and medical
condition.
• Home Nursing Service –
attaches a nurse to aid in the social, functional, financial and nursing
care of the patient. Together with the doctor’s assessment, a care plan is
determined. The care plan involves:
- Continual assessment of the
health and medical condition.
- Nursing procedures e.g. wound, urinary tube, feeding tube, & airway care.
Injections & blood draw.
- Education of patients and caregivers e.g. prevention of bed sores, proper
feeding techniques, diabetic & dementia care, etc.
• Home Help & Support -- simple
home modification, counselling, application of financial aid for milk
supplies, wound care products, etc. for needy patients.
